In this post we are going to describe a case solved by Dr. Diego Peydro, in which a patient with severe crowding and dental agenesis was successfully treated without the need for surgery or extractions, using clear aligners alone.
This was a patient who, since his teens, had had severe crowding, which affected not only the look of his teeth but also his chewing function and his overall oral health. He also had dental agenesis, that is, the congenital absence of one or more teeth, which made the case even more complex.
How did we solve this case?
At the first appointment, various assessments were carried out in order to fully understand the patient’s situation.
Dr. Peydro decided that the best approach for this case would be clear aligners. That method would allow the patient to correct his dental problem discreetly and effectively, without the need for surgery or extractions. The main objectives were:
- Correcting the severe crowding: creating enough space in the arch to align every tooth correctly.
- Managing the dental agenesis: repositioning the existing teeth to cover the gaps in a way that both looked good and worked well.

The treatment was carried out in several carefully planned stages:
Stage 1: expansion and initial alignment
During the first year, the patient wore aligners aimed at expanding the dental arches and starting to align the most crowded teeth. That process involved controlled micro-movements that allowed the teeth to be moved without causing the patient damage or significant discomfort.
Stage 2: repositioning the baby teeth
Once enough space had been created, we worked on repositioning the baby teeth so as to solve the agenesis the patient had. Once that treatment was over, the patient decided to have esthetic dentistry treatment to improve the look of his smile
The treatment, which lasted less than two years, was a resounding success. At the end of the process the patient had a completely aligned smile that worked well and looked good. The specific results were:
- Perfect alignment of both arches: the teeth were correctly positioned without any extractions.
- Esthetic closure of the gaps from the agenesis: repositioning the teeth covered the gaps effectively, achieving a natural appearance.
