Most patients with old crowns come to our clinic after seeing how, over time, the join between the crown and the gum starts to show, leaving a dark line around the tooth.

Today, replacing old crowns with porcelain crowns or veneers is one of the esthetic treatments we carry out most at the clinic, since the materials used in the laboratory are far more esthetic than they used to be.
These days, when we think about restoring an esthetic area, we consider doing it with metal-free porcelain. When the whole structure of the crown is porcelain alone, the way it behaves optically with light is more similar to enamel.
Laboratory technicians can produce work that is more translucent and transparent and more like natural teeth, achieving more spectacular results.
Replacing old crowns with porcelain crowns
Today we are presenting the case of an adult patient who had two porcelain-fused-to-metal crowns on her upper central incisors. As you can see, the crowns no longer met today’s standards of excellence for dental crowns.
As we can see in the image above, the patient had a smile that was far from attractive, since:
- The color of the crowns was neither the same as nor similar to that of her natural teeth
- The crowns were very opaque and did not let light through
- The proportion of the crowns was not right compared with the natural teeth
- Their square shape was damaging the gum
- They were large
- The shape of the gum was flat and unnatural, giving her a gummy smile that did not look good
- The color of her natural teeth was very dark
The treatment plan for the patient
After studying the case, our Dr. Marta Peydro decided on a treatment plan with 3 steps:
Gingivoplasty
The gums needed harmonious treatment so that, for reasons of both health and appearance, every tooth had the gum at the right level. To do that we carried out surgery in the upper jaw on both the gum and the bone around the teeth, so that this tissue would have the right volume, size and shape for the patient’s teeth.
Teeth whitening
Since we were going to carry out treatment with high-esthetic porcelain, we wanted to improve the overall color of her teeth so that the final restorations would also be lighter and brighter.
To do that we carried out combined in-practice and at-home whitening, with 37.5% high concentration hydrogen peroxide at the clinic and 16% carbamide peroxide applied at home with custom trays.
Replacing old crowns with porcelain crowns
Replacing the old porcelain-fused-to-metal crowns on the central incisors with two porcelain crowns, and making porcelain veneers for the two lateral incisors to give them more volume, length and size so that all 4 incisors were on the same plane, without the central incisors dominating by size as they did with the old crowns.
The final result is a healthier, more harmonious smile, pleasant to look at, younger, brighter and completely natural, which delivers:
- Gummy smile esthetics: we have reduced the gummy smile as far as possible, giving the gum a natural shape and correcting the uneven levels and the excess gum the patient showed when she smiled.
- Color esthetics: matching the color of our 4 porcelain restorations to that of the whitened natural teeth.
- Size esthetics: we have followed the right proportions so that our veneers and crowns blend naturally with the natural teeth around them.
- Texture esthetics: the porcelain follows the same surface pattern and transmits light in the same way as natural teeth.
