Clear aligners and whitening in a patient with severe discoloration
Today we are showing the case of a patient who came to Clínica Peydro hoping to recover the natural color of his teeth and improve the look of his smile. Despite being very young, he did not like his smile and wanted to feel more comfortable and more confident when smiling. He had decided it was time to enjoy a beautiful smile and to go for clear aligner treatment and whitening in order to get one.
The patient had different shades of color on each of his teeth. He was aware of it and, seeing that the situation was only getting worse, he decided to come to us to get the smile he wanted so much.

Before the clear aligner treatment and the whitening
Before starting active treatment, we first made a proper diagnosis of the problems he had:
- There was a discrepancy in the color of his teeth
- 3 of the teeth were darker than the rest
- The patient had a wide smile and showed all his teeth
- The position of his teeth was not right. He had a significant malocclusion that we would have to solve, both to improve the look of his smile and to improve his bite and chewing function.
After studying his case meticulously with radiographic and clinical examination, the X-rays showed us that the 3 teeth that were darker than the rest (upper right central incisor, upper left lateral incisor and lower left lateral incisor) had never had root canal treatment (nerve treatment). Even so, those teeth tested negative for vitality, so we concluded that they must have suffered some kind of trauma that was never treated.

To solve all the problems the patient had, multidisciplinary treatment was carried out consisting of orthodontics and teeth whitening, led by Dr. Peydro and Dr. Marta Peydro respectively.
Orthodontic treatment
Before the esthetic dentistry phase, we had to carry out proper nerve treatment on the teeth that tested negative for vitality and were darker in color, because of the necrosis in the nerve of those teeth.
Dr. Diego Peydro carried out orthodontic treatment, following his advanced orthodontic protocols, to improve the position of the teeth, correct the crowding and the discrepant tilts and widen the arch of the smile. All of it without needing any extractions. That allowed us not only to improve the look of the smile but also to provide stability, function and a correct bite.
Teeth whitening treatment
Once the orthodontic treatment was over, esthetic dentistry treatment was carried out using advanced whitening techniques, led by Dr. Marta Peydro. Dr. Peydro used two whitening techniques for this:
On the one hand, as you can see, the three teeth that were darker were treated from the inside, with internal whitening first, using 35% carbamide peroxide.
The rest of the teeth, including these, were treated with a combination of in-practice treatment using high concentration hydrogen peroxide and at-home whitening with 16% carbamide peroxide.
As you can see, we have managed to:
- Correct the position of the teeth thanks to the orthodontic treatment
- Achieve harmony in the color of every tooth
- Achieve overall whitening of all the teeth treated
- Achieve a very bright smile
As you can see, the patient now has a spectacular smile thanks to the perfect combination of two entirely conservative treatments: orthodontics and teeth whitening.
