A complex malocclusion case solved without surgery
Many of our patients come to the clinic with a closed diagnosis, having been told that their case can only be solved with bone surgery.
That was the diagnosis in the case we are presenting today: a young patient with a very significant skeletal facial asymmetry, deformed bones and a bite with both class II and class III malocclusion.
Here is how Dr. Diego Peydro successfully resolved this case without the need for surgery.

Skeletal facial asymmetry
The patient had a skeletal facial asymmetry in which his whole face was deviated; in the images below we can see how his nose and chin were shifted to the right. The case was also made worse by a malocclusion that stopped the jaw meeting correctly.
On one side, this patient had a class II malocclusion and on the left side a severe class III malocclusion, as well as an open bite, that is, the upper teeth did not cover the lower ones.
On top of that, it was causing a problem in the temporomandibular joint.

Planning the case
After a thorough analysis, Dr. Peydro decided to treat the case with clear aligners, without resorting to surgery to solve the severe malocclusion.
The aim of the treatment was to improve the bite —that is, to resolve that malocclusion— and to center the lower jaw in relation to the upper one, reducing the appearance of facial deformity.
As well as the clear aligners, we used microscrews.
The results?
A bite that fits together properly, resolving the class II malocclusion. And, in the class III malocclusion, an excellent class I result.
Most importantly, we have managed to close the bite and balance out the facial asymmetry, giving a stable occlusion and avoiding an operation. And we have improved the look of this young man’s face.
Our message is: less is more. Thanks to advances in medicine and technology we can turn to techniques that give us excellent results without the surgery so many people fear.
