
In this article we look at a case solved by Dr. Diego Peydro in which a patient with an open bite and a class II malocclusion was treated with clear aligners alone, without the need for surgery or extractions. The patient came to Dr. Diego Peydro’s practice with two main concerns: an open bite and a class II malocclusion. These problems affected not only the look of his smile but also his chewing function and his overall oral health. The patient was also worried about possibly needing surgery or extractions, and was looking for a less invasive solution.
Dr. Peydro designed a comprehensive treatment plan using clear aligners alone. That approach would correct both the open bite and the class II malocclusion without resorting to invasive methods such as surgery or extractions. The treatment plan set out the following objectives.
- Closing the open bite: aligning the teeth so that they meet correctly on biting.
- Correcting the class II malocclusion: repositioning the teeth and jaws to achieve a proper bite.
- Improving chewing function: ensuring an efficient, functional bite.

The treatment was structured in several stages, each designed to address different aspects of the open bite and the class II malocclusion.
During the first few months, aligners were used to start aligning the teeth and slightly expanding the dental arches. That initial process helped reduce the open bite and prepared the arches for more complex movements.
Aligners with specific features were then used to gradually move the upper teeth back and the lower teeth forward. That process involved using intermaxillary elastics together with the aligners in order to achieve the necessary movements.
Aligners were designed to intrude the back teeth and extrude the front teeth, levelling the occlusal plane and reducing the gap between the upper and lower teeth, thereby solving the open bite the patient had
