Hello everyone. Today I am presenting the case of a 16-year-old girl who came to our clinic to correct a narrow smile together with a very severe class II malocclusion. She was a very shy girl who felt embarrassed about smiling because of her bite.

As you can see in the photographs, she had a narrow smile. And if you look at the profile view, you will notice a large discrepancy between the upper jaw and the lower jaw.






In this same photograph we can see how the mandible sits well behind the upper jaw, something the X-ray confirms as well.

As you can see on the X-ray, which matches the patient’s profile exactly, we not only had to move the teeth: we had to move them in a way that brought both jaws into a correct relationship with each other and so corrected the narrow smile.
As a curious detail, this patient had been fitted with braces some years earlier and, as you can see in the initial intraoral photographs above, the result achieved had not been satisfactory.
She had been left with a class II malocclusion and also with enormous instability, which is why the case had relapsed completely.
When we saw the patient, the treatment plan we proposed was to work with Invisalign. She had already been through treatment with braces and did not want to go through the same thing again. On top of that, we knew we would achieve the most spectacular results using clear aligners and, more importantly, without making the patient suffer, while correcting her narrow smile.

The clear aligner treatment lasted around a year and a half, during which the patient wore aligners only. At one point in the treatment we used intermaxillary elastics to coordinate the upper arch with the lower one.
By the end of the treatment we had resolved the narrow smile and the severe class II malocclusion and achieved a perfect occlusion, with the teeth meeting properly and complete stability. We now know that this result will hold.





As we always say, the best stability in a case comes from having a correct occlusion. On top of that, this girl now has a spectacular, full smile, a far more attractive lower third of the face and lips with much more support. We have given her mouth health, and she will enjoy it for the rest of her life.
